GHC is proud to announce a new and improved Explanation of Benefits (EOB) form (PDF, 1.1MB).
The new, reader‑friendly format will assist GHC members in understanding their benefits. Some features of the new EOB include:
- Logical and visually appealing layout
- Easy‑to‑understand language
- Claims process clearly defined
- Patient’s responsibilities clearly defined
Attached is a sample of the new form as well as an “EOB Key” that will explain the features of the new form in greater detail.
