This is an assessment instructors can hand out to students mid way through the class to get feedback and make adjustments to enhance learning in the classroom. The assessment should be anonymous so students feel comfortable enough to give honest input. The categories used for question #1 should be designed to fit the learning experiences used in your classroom. The categories used in this assessment are examples.

Again with any assessment, if an instructor asks students for their feedback, the instructor should respond to the students concerning their feedback by making adjustments and/or discussing the information he/she learned from the assessments.
