UPDATE - Accident/Incident Report Form

There have been a few changes worth noting to the MATC Accident/Incident Report Form (PDF: 52KB).

  1. The form has been consolidated into one report form, for ease of use and processing. This form is intended to be used by all individuals (staff, students and visitors) for reporting purposes.
  2. Completed forms are to be forwarded now to the Environmental Health and Safety (EH&S) Office for processing and follow-up. This will help streamline risk management activities.
  3. A written procedure describing the process and responsibilities is online at the EH&S website.

The intent of one form is two fold:

  1. Make the process less confusing to report by students and staff.
  2. Provide and better process for reporting accidents and ensuring timely investigations.

Instructions for completing the form:

  1. All school-related accidents/incidents (student, visitor, athlete) require Sections I and II of the MATC Accident/Incident Report to be completed by the injured person. If unable to do so, the person supervising the activity is to complete the accident report.
  2. In all cases, the supervisor of the activity is to complete Section III on the reverse side, review the report for completeness and accuracy, sign and forward to the Environmental Health & Safety Manager within 24 hours of the accident/incident.
