Deadline for Outstanding Employee Award/Team nominations

MATC Outstanding Employee Award/Team Award Nomination Form 2006 – 2007Deadline Extended:
Due to recent reorganization efforts and our change to announcing Outstanding Team and Employee Awards in October, the deadline for submitting nominations has been extended.
All completed nomination forms must be received by a member of the committee or in the committee chair’s mailbox at Truax no later than 4 p.m. on Monday, September 24, 2007. You may submit and/or sign more than one nomination form but you must use separate forms for each nominee. Photocopies of the form are acceptable; however, photocopied signatures are not. The form is available online; but it is not considered a complete nomination form until the original signature page is received.
Online nomination forms must meet the same deadline as the hardcopy forms. Submit online forms to Valerie Weihman-Rock.
