MATC Matters

As the year comes to a close, remember to turn in your receipts for reimbursement of health & wellness services administered during the 2008 calendar year.

GHC Members:
Receive up to $100 per individual plan or $200 per family plan per calendar year. Deadline for reimbursement submission is January 15.

Visit GHC’s Website to view eligible services and to print the Wellness Reimbursement Form. Send your form to GHC-SCW Marketing Department, Attn. Wellness Reimbursement, P.O. Box 44971, Madison, WI 53744-4971.

WPS Members
Receive up to $200 per employee and $200 per spouse per calendar year for participation in a number of health and wellness services. Deadline for reimbursement is 16 months from the time of service. 

Services are reimbursed at 30%, 60%, or 90%. For a breakdown of eligible services, read the Wellness Benefit portion of the WPS Active Plan.

Print the WPS HealthSense Reimbursement Form, complete the form, attach receipts and mail to: WPS, P.O. Box 8190, Madison, WI 53708 or fax to Tim Ottosen, WPS Customer Service Consultant at 608-223-5861.
